Notary Jurat Form

This notary jurat form is used to certify that the person named within personally appeared before a notary public, was identified, and signed the document in the notary’s presence. The individual also swore or affirmed under oath that the statements in the document are true and correct. The notary public confirms the identity of the individual either through personal knowledge or by reviewing satisfactory evidence, and administers an oath or affirmation regarding the truthfulness of the contents of the document.
